This is something I don't quite understand.

Apparently there were two output modules available for the Meridian 101B pre-amp - one that was 75 Ohm and simply said 'output' on the cover and one that was balanced and had 'balanced' on its cover.

My (simple) understanding was that the 101B was balanced output by default. I didn't realise that a balanced module was required - an I am confused as to why this was the case.

Anybody care to share any light?

As I understand it, the difference between the 101 and 101B was the existence of a (high sensitivity) moving coil module on the latter. My (very old) 101 instructions talk very clearly about it being preferable to use balanced output between a 101 and (in my case) M2s. There is also something about 75ohm terminators for the RCA phono in (on the M2s) if using balanced leads other than those supplied by Meridian, but don't ask me anything about it as I don't recall any detail. I used to have M's leads but threw them away years ago as I found coax leads less trouble and perfectly adequate (for me). Sounds like the M30s had switches to overcome any need for terminators etc. You can find the old instruction leaflets in various places online.
