Dolby specs assume that everyone can hang box speakers aimed like laser beams at the main listening position. Most speakers designed for in wall or in ceiling use have a sixty degree or better dispersion rating so being out of the death ray is not that much of a detriment. It's not like being a bit off axis means you don't hear the speaker or that it doesn't sound good. If that were the case, only one person in the theater would get the best experience.
