It can happen. That is why we went to polarized plugs in the first place. Audio ground/neutral doesn't have to be to be tied to the chassis.
Shawn Neutral is never, ever intentionally tied directly to ground. In fact, if you plug it into a ground faulted circuit it will trip as fault.
